Largest Review Yet Finds Nicotine Vaping Beats Patches and Gum, but Long-Term Risks Remain Unclear

Nicotine e-cigarettes appear to be one of the most effective tools currently available for people trying to quit smoking, outperforming traditional nicotine replacement therapies such as patches and gum. That is the conclusion of a major evidence review, although researchers stress that important caveats remain.

The findings suggest that smokers who switch to regulated nicotine vaping products are more likely to stop smoking than those relying on older methods or no cessation aid. However, the long-term health effects of vaping remain less certain, making the balance between effectiveness and potential risks an important consideration.

What the new review found

The review, published in the Cochrane Database of Systematic Reviews, combined data from 80 randomized controlled trials involving almost 30,000 smokers worldwide. Cochrane reviews systematically assess the quality and consistency of available evidence to determine how confidently conclusions can be drawn.

Researchers found high-certainty evidence that, for every 100 smokers, using nicotine e-cigarettes leads to around four additional successful quitters compared with nicotine replacement therapies such as patches or gum. Compared with non-nicotine e-cigarettes, nicotine vapes produced about two additional quitters per 100 people.

Lead author Nicola Lindson of the University of Oxford said many smokers have tried to quit repeatedly without success, making a wider range of effective options crucial. She noted that while nicotine vapes are not risk-free, they are substantially less harmful than continuing to smoke tobacco for people who already smoke.

Only regulated vaping products assessed

The review focused on nicotine e-cigarettes used in controlled research settings rather than illicit or counterfeit products. This distinction is important because the composition and quality of vaping devices and liquids can vary substantially outside regulated markets.

Health policy expert Jamie Hartmann-Boyce of the University of Massachusetts Amherst has emphasized that risks may differ depending on the products being used. Unregulated or illicit products can introduce additional hazards that are not necessarily represented in clinical trials of smoking cessation.

In the trials examined, participants used e-cigarettes as a replacement for combustible cigarettes, sometimes alongside behavioral support such as counseling or structured quit programs. Behavioral support can itself improve the chances of successfully quitting smoking.

Benefits versus unknown long-term risks

The review supports evidence that nicotine e-cigarettes expose users to fewer harmful substances than combustible cigarettes. Traditional smoking remains an exceptionally harmful form of nicotine consumption and causes millions of deaths worldwide each year, including deaths associated with secondhand smoke exposure.

Despite that contrast, researchers emphasize that vaping is not harmless. E-cigarettes deliver nicotine and other substances through inhaled aerosols, and scientists continue to investigate how prolonged exposure may affect the lungs, cardiovascular system and other organs.

The trials included in the review did not provide evidence of substantial differences in serious adverse events between nicotine e-cigarettes and the comparison groups. However, the number of such events was generally low, and most studies were not designed to establish the health consequences of vaping over decades.

Larger and longer-term studies will therefore be needed to characterize potential chronic risks more precisely, particularly because many diseases associated with inhaled exposures can take years to develop.

Unanswered questions on addiction and youth

The review primarily addresses e-cigarettes as smoking-cessation tools for people who already smoke. It does not establish that vaping is safe or beneficial for people who do not smoke, nor does it resolve how former smokers who continue vaping should eventually stop using nicotine.

These distinctions are particularly important in the context of youth vaping. E-cigarette use among adolescents raises separate concerns about nicotine dependence and exposure to potentially harmful substances and should not be conflated with vaping as a cessation intervention for adults who smoke.

Separate research has raised concerns about exposure to toxic metals among young people who vape. Observational studies have also found associations between adolescent e-cigarette use and subsequent combustible cigarette smoking, although such associations do not by themselves establish that vaping directly causes later smoking.

Co-author Chris Bullen of the University of Auckland has emphasized that evidence concerning e-cigarettes as cessation aids should be distinguished from questions surrounding vaping among young people and people who have never smoked. Those issues require separate research and evidence reviews.

Implications for doctors and policymakers

For clinicians, the review strengthens the evidence for nicotine e-cigarettes as one potential cessation option for adults who smoke, particularly when other approaches have not worked. The appropriate strategy can depend on individual preferences, previous quit attempts and the cessation treatments available in a particular country.

For policymakers, the challenge is more complicated. Regulations must balance access to potentially effective cessation tools for established smokers against efforts to discourage uptake among young people and non-smokers. Countries have consequently adopted substantially different approaches to e-cigarette availability and regulation.

The review was supported by Cancer Research UK, the US National Cancer Institute and the FDA Center for Tobacco Products. Because evidence in this field continues to develop, the Cochrane review is regularly updated as new studies become available.

For adults who already smoke, the evidence indicates that nicotine e-cigarettes can increase the likelihood of successfully quitting combustible cigarettes compared with nicotine replacement therapy. At the same time, vaping should not be considered harmless, and its long-term health effects remain less well understood than the extensive risks associated with conventional smoking.
