Regular blistering sunburns during childhood and adolescence may leave a permanent mark on health, not just on memory. A new meta-analysis in JAMA Dermatology links these so-called life-changing sunburns to a significantly higher risk of cutaneous squamous cell carcinoma, a common form of skin cancer.
The review, led by dermatologist Isaac Weber of the University of Texas MD Anderson Cancer Center, pooled data from 17 separate studies. In total, the analysis examined more than 321,000 people, comparing those with painful, blistering or severe sunburns with individuals who had little or no history of serious burns.
What the new research found
The researchers reported that people with a medium frequency of severe sunburns had 51 percent higher odds of developing cutaneous squamous cell carcinoma. For those with a high frequency of such burns over their lifetime, the odds climbed by 69 percent compared with people who reported no severe sunburns.
Stronger associations emerged when the team focused on childhood and teenage years. In some studies, frequent severe sunburns early in life were linked to as much as a threefold increase in the odds of later squamous cell carcinoma. Even a handful of bad burns appeared to nudge risk upward.
The team tried to distinguish the effect of discrete sunburn episodes from overall time spent in the sun. They noted that this is difficult because people who burn often are usually exposed to more ultraviolet radiation in general. Where possible, the underlying studies adjusted for age, sex, skin type and sensitivity.
Why severe sunburns matter
Experts say the findings support a long-held biological explanation. Ultraviolet radiation from sunlight damages DNA in skin cells, and the body must repair those lesions. When repairs are imperfect, mutations can accumulate, eventually driving cells toward cancer.
Oncologist Adrienne Victor from the University of Rochester Medical Center, who was not involved in the study, explained that the pain of a sunburn fades but the molecular damage can linger. Over years, repeated injuries and repair errors can contribute to the development of skin cancers, including squamous cell carcinoma.
Children may be especially vulnerable because their pigment-producing cells are still maturing. They also tend to spend more time outdoors and may use sun protection inconsistently, which together increase the chances of intense burns during critical developmental periods.
Other skin cancers and UV risks
Previous meta-analyses have connected sunburn history to other major skin cancers such as melanoma and basal cell carcinoma. Across these studies, a consistent pattern appears: the more frequent and severe the burns, particularly in youth, the greater the lifelong cancer risk.
Genetic factors can further modify this risk, especially for people with very fair skin, a tendency to freckle or a family history of skin cancer. Lifestyle choices matter as well. Cigarette smoking has been associated with certain skin cancers, and indoor tanning exposes skin to concentrated artificial ultraviolet radiation.
Research published in recent years suggests regular tanning bed use may be even more damaging per session than typical outdoor sun exposure. Several studies have tied indoor tanning to higher melanoma rates, prompting many countries and US states to restrict use among minors.
Prevention strategies and ongoing debates
Despite lingering myths, dermatologists stress that sunscreen is a key tool for reducing sunburn and long-term cancer risk. Large population studies have found that consistent use of broad-spectrum, high-SPF products is associated with fewer cases of melanoma and other skin cancers.
Some people worry about potential side effects from sunscreen ingredients, but current evidence points far more strongly to ultraviolet radiation as the main driver of skin cancer. Regulators in the United States and Europe continue to review filters for safety, and in recent years new UV-filtering ingredients have gained approval after detailed testing.
Beyond sunscreen, experts recommend practical measures such as avoiding the midday sun, wearing tightly woven clothing, seeking shade and using wide-brimmed hats and UV-blocking sunglasses. Public health campaigns, including Australia’s long-running Slip-Slop-Slap slogan, have demonstrated that simple habits can reduce sunburn rates.
For adults who have already accumulated burns earlier in life, doctors emphasize that it is never too late to lower future risk. Regular skin checks, both self-examinations and clinical screenings, can help catch precancerous lesions and early cancers when they are most treatable.
The authors of the new meta-analysis caution that their conclusions are limited by the quality of available data, which often depend on people remembering sunburns decades later. Still, they argue that the evidence is strong enough to justify aggressive prevention, especially for children and teenagers who still have time to avoid adding more damage.
As summer seasons grow longer and heat waves become more intense in many regions, dermatologists warn that unprotected time in the sun carries cumulative consequences. They say the message from the latest research is clear: while we cannot change past sunburns, carefully limiting new ones may substantially reduce the odds of developing skin cancer later in life.
