Artificial sweeteners such as aspartame or xylitol are often accused of harming the brain and speeding up cognitive decline. Yet growing evidence suggests that sugar itself, especially in excess, poses a far greater and better documented threat to long-term brain health.
Australian epidemiologist Gideon Meyerowitz-Katz has reviewed widely publicized studies that linked sweeteners with faster brain ageing. His conclusion is that many alarming headlines were driven more by weak methods and misinterpretation than by solid science.
Current evidence does not consistently show that approved artificial sweeteners directly damage the human brain when consumed at normal levels. By contrast, chronic overconsumption of added sugar contributes to obesity, insulin resistance and type 2 diabetes, conditions strongly associated with vascular damage and cognitive impairment later in life.
Why one high-profile study misled
Media panic intensified after a 2025 report based on Brazilian ELSA cohort data claimed that people who consumed the most sweeteners showed worse cognitive performance over eight years. On the surface the analysis appeared robust, which helped its rapid spread.
The research relied on food frequency questionnaires, asking participants to recall what they had typically eaten over the previous 12 months. Such surveys are known to be imprecise, because people badly remember their diet and often underestimate or overestimate specific foods.
Scientists then assumed that eating patterns stayed essentially unchanged over the full eight-year follow-up. In reality, many people alter their diet even within a year, and over longer periods weight changes, illness or lifestyle shifts can drastically reshape what they consume.
Another weakness was grouping very different substances into a single exposure. Natural sugar alcohols, rare sugars and synthetic sweeteners were all treated as if they had the same biological effect, although they differ in how the body absorbs and metabolizes them.
The researchers also failed to fully address reverse causality. People already living with obesity, diabetes or cardiovascular disease are more likely to switch from sugar to sweeteners, but it is these underlying conditions, not sweeteners, that strongly raise the risk of stroke and dementia.
Independent experts later pointed out statistical errors and inconsistencies in the paper. In the full analysis set, relegated to an appendix, there was no significant association between sweetener intake and brain ageing at all, undermining the dramatic interpretation carried in headlines.
What broader research shows so far
Despite public suspicion, artificial sweeteners are among the most heavily tested food additives worldwide. Regulatory bodies such as the EFSA and FDA have repeatedly concluded that approved sweeteners are safe at normal intake levels for the general population.
Large observational studies and randomized trials suggest that replacing sugar-sweetened beverages with diet versions can support weight loss or maintenance. Reduced calorie intake and lower post-meal glucose spikes help protect blood vessels, including those that supply the brain.
Some research has explored potential links between very high sweetener use and cardiovascular risks, but findings remain mixed and often confounded by pre-existing health problems. So far, consistent and strong evidence of direct neurotoxicity in humans is lacking.
By contrast, the harm caused by chronic overconsumption of added sugar is firmly established. Excess sugar contributes to obesity, fatty liver disease, insulin resistance and type 2 diabetes, all of which are major drivers of vascular damage and cognitive impairment in later life.
High sugar intake promotes systemic inflammation and stiffening of blood vessels, including cerebral arteries. These changes increase the likelihood of stroke and vascular dementia and can also worsen the course of Alzheimer’s disease through metabolic stress.
How to protect your brain in practice
Health authorities now recommend limiting free sugars to no more than about 10 % of daily energy intake, and ideally closer to 5 %. For someone on a typical diet this means sharply reducing sugary drinks, sweets and ultra-processed snacks.
For many people, replacing sugar-sweetened soft drinks with zero-calorie alternatives is a realistic first step. Doing so can cut thousands of calories per month, reduce weight gain and ease the metabolic strain associated with elevated blood glucose and insulin.
However, sweeteners are not a nutritional cure-all. Some compounds may briefly stimulate appetite or alter gut comfort in sensitive individuals, and they can maintain a strong preference for intensely sweet tastes, making a less processed diet harder to adopt.
Experts therefore advise using sweeteners as a transitional tool rather than the foundation of daily nutrition. Emphasising whole foods, high-fibre carbohydrates, healthy fats and regular physical activity brings far more powerful long-term benefits for brain and heart health.
Comparing the two options directly, current scientific evidence weighs clearly against excess sugar and far less against approved sweeteners. If a can of diet soda instead of a sugary drink helps you maintain a healthy weight and stable metabolism, your brain is more likely to benefit than suffer over time.
