Researchers Found GLP-1 Drugs Were Linked to Better Mental Health in Nearly 100,000 Adults

GLP-1 drugs such as semaglutide, marketed as Ozempic, Wegovy and Rybelsus, may improve not only physical health but also mental well-being, according to a large new study published in The Lancet Psychiatry. Researchers found that people taking GLP-1 receptor agonists experienced fewer psychiatric hospital visits, lower rates of depression and anxiety, fewer substance use problems, and reduced sick leave related to mental health.

The analysis, conducted by researchers from the University of Eastern Finland, Karolinska Institutet in Sweden and Griffith University in Australia, examined nationwide health records to investigate whether medications used to treat obesity and type 2 diabetes might also influence psychiatric outcomes in everyday clinical practice.

Mental health improvements alongside weight loss

Obesity and type 2 diabetes are both associated with a substantially higher risk of depression, anxiety and other psychiatric disorders. At the same time, people living with serious mental illness are more likely to develop metabolic diseases, creating a complex two-way relationship that has long challenged clinicians.

To explore this connection, the researchers analyzed healthcare records from nearly 100,000 adults in Sweden between 2009 and 2022. More than 20,000 participants had used GLP-1 receptor agonists, allowing investigators to compare periods when the same individuals were taking the medication with periods when they were not.

During treatment with semaglutide and related GLP-1 drugs, psychiatric hospital visits and mental health-related sick leave were 42 percent lower than during periods without treatment.

The researchers also found a 44 percent lower risk of depression and a 38 percent lower risk of anxiety disorders while participants were receiving GLP-1 therapy.

Benefits extended to addiction and suicidal behavior

The positive associations extended beyond mood disorders.

Hospital admissions and sickness absence related to substance use disorders were 47 percent lower during periods of GLP-1 treatment. These findings are consistent with previous research suggesting that GLP-1 receptor agonists may reduce alcohol consumption and lower the risk of alcohol use disorder.

The researchers also observed a lower risk of suicidal behavior among people taking GLP-1 medications.

Although suicide attempts remained relatively rare overall, the observed reduction is notable given previous concerns about mental health risks among people living with obesity and diabetes.

Co-author Mark Taylor of Griffith University noted that earlier registry studies had already suggested lower rates of alcohol-related problems among GLP-1 users. Because alcohol misuse frequently occurs alongside depression and anxiety, the researchers expected some mental health benefits, although the size of the association exceeded their expectations.

Possible effects on the brain

GLP-1 receptor agonists were originally developed to improve blood sugar control, slow stomach emptying and promote weight loss.

However, GLP-1 receptors are also widely distributed throughout the brain, including regions involved in reward, motivation and stress regulation.

Lead author Markku Lähteenvuo of the University of Eastern Finland emphasized that the registry study cannot determine exactly how these medications influence mental health.

Several explanations may contribute simultaneously, including improved blood glucose control, weight loss, better physical health, increased body confidence, reduced alcohol consumption and possible direct effects on brain reward circuits.

Previous animal studies and smaller human experiments have shown that GLP-1 drugs can reduce reward responses to both food and alcohol. This may help explain why many patients report diminished cravings not only for highly processed foods but also for addictive substances while taking semaglutide-based medications.

Important limitations

The researchers stress that the findings do not prove GLP-1 medications directly improve mental health.

Because the study was observational, people who remained on treatment may differ from those who discontinued therapy in ways that also affect psychiatric outcomes. Improvements in physical health alone may also relieve psychological distress independent of any direct action of the drugs on the brain.

The results nevertheless add to a growing body of evidence suggesting that GLP-1 receptor agonists may have broader effects beyond metabolism.

Earlier clinical trials have produced mixed findings regarding mood, with some studies reporting improvements while others found little change. Many of those trials, however, were smaller or followed participants for relatively short periods.

The new registry analysis provides longer-term real-world evidence but still cannot establish cause and effect.

What comes next

Regulatory agencies in Europe and the United States continue to monitor reports of suicidal thoughts among some GLP-1 users.

Interestingly, the Swedish study found an overall reduction in suicidal behavior during treatment periods, highlighting the need for further research to better understand these apparently conflicting signals.

The researchers say randomized controlled trials specifically designed to measure psychiatric outcomes are now needed.

If future studies confirm these findings, GLP-1 receptor agonists could eventually have applications beyond obesity and diabetes, potentially contributing to the treatment of certain mood disorders and addictions.

For now, clinicians recommend continuing to monitor both physical and mental health when prescribing GLP-1 medications. Patients are also advised not to stop or change treatment without consulting their healthcare provider, as metabolic and mental health are closely interconnected.
