Tattoos have been part of human culture for thousands of years, serving as symbols of identity, spirituality, and personal expression. Although tattooing is now more common than ever, researchers are still working to understand how tattoo ink interacts with the body and what long-term health effects it may have.
When tattoo ink is injected into the dermis, the layer of skin beneath the surface, the immune system immediately identifies the pigment as a foreign substance. Immune cells move to the area and attempt to remove the ink particles. However, because many of these particles are too large to be eliminated, they remain trapped in the skin.
The permanence of tattoos is largely due to this process. The pigment becomes enclosed within dermal cells, including immune cells known as macrophages. As these cells naturally die and are replaced, newly formed macrophages take up the released pigment, allowing the tattoo to remain visible for decades despite the skin’s continuous renewal.

Modern Tattoo Inks Contain Complex Chemical Mixtures
Tattoo inks contain far more than simple coloring agents. Most formulations include pigments, liquid carriers such as water or alcohol, preservatives that help prevent microbial growth, and small amounts of impurities that may not always appear on product labels. Many pigments used in tattoo inks were originally developed for industrial applications rather than for use inside the human body.
Studies analyzing commercial tattoo inks have identified trace amounts of metals including nickel, cobalt, chromium, and, in some cases, lead. At high concentrations, these substances have been linked to organ damage, neurological disorders, and an increased risk of cancer. Even at much lower levels, they may trigger allergic reactions or heightened immune responses in susceptible individuals.
Colored inks—particularly red, yellow, and orange pigments—are more frequently associated with allergic reactions and chronic skin inflammation. Some people develop persistent itching, swelling, or granulomas, which are small inflammatory nodules that form when the immune system attempts to isolate substances it cannot remove.
Some of these reactions have been linked to metal salts and azo pigments that produce bright colors. These pigments are also widely used in textiles and plastics. Under certain conditions, including exposure to sunlight or laser tattoo removal, azo pigments can break down into aromatic amines, some of which have caused genetic damage and cancer in laboratory studies.
Black tattoo inks may contain polycyclic aromatic hydrocarbons, a group of chemicals also found in soot and vehicle exhaust. Several compounds within this group are classified as carcinogenic. Although the concentrations present in tattoo inks are generally low, researchers continue to investigate whether decades of exposure within the skin and lymphatic system could have long-term health consequences.
Scientists Continue to Study Possible Cancer Risks
Evidence linking tattoos directly to cancer in humans remains limited, and existing research has produced mixed results. One observational study published in 2023 reported that people with tattoos had a 29% higher relative risk of melanoma compared with individuals who did not have tattoos.
Other studies, however, have not identified a clear association. Researchers note that factors such as skin type, sun exposure, and lifestyle may also influence cancer risk, making it difficult to isolate the role of tattoo ink.
Laboratory and animal research has shown that some tattoo ink components can damage DNA or contribute to tumor development under experimental conditions. However, these findings cannot be directly applied to people, and scientists stress that more long-term research is needed.
As tattoos become increasingly common and many people receive larger or multiple tattoos at younger ages, researchers say continued monitoring and stronger safety standards will become increasingly important.
Tattoos May Present Additional Risks for Some People
Because tattoo pigments remain in the body and continue to interact with the immune system, people with autoimmune diseases or weakened immune systems may require additional consideration before getting tattooed. Persistent local inflammation has the potential to affect surrounding tissues and could interact with existing immune conditions, although current evidence remains limited.
Tattooing also temporarily breaks the skin’s protective barrier, creating an opportunity for infection. If equipment is not properly sterilized or hygiene practices are inadequate, bacteria such as Staphylococcus aureus can cause infections, while viruses including hepatitis B and hepatitis C may be transmitted. Rare infections caused by atypical mycobacteria have also been reported.
Because tattoo regulations differ widely between countries and regions, safety standards can vary considerably. Public health experts recommend choosing licensed tattoo studios, asking about sterilization procedures and single-use needles, and requesting information about the inks being used.
Regulation of Tattoo Inks Is Evolving
In recent years, regulators have increased scrutiny of tattoo inks. The European Union has restricted a number of pigments and chemical substances under its REACH regulations because of concerns about possible cancer and allergy risks. Manufacturers are also being encouraged to provide more detailed information about the ingredients used in their products.
In many other parts of the world, including some regions of North America, oversight remains less consistent, and tattoo inks may not undergo the same level of testing required for cosmetics or medical products. Researchers argue that stronger regulation and greater transparency are needed given the widespread and permanent nature of tattooing.
