Getting enough sleep is essential for overall health, but emerging evidence suggests that one particular stage of sleep may play a critical role in protecting the aging brain. Recent research indicates that the loss of deep slow-wave sleep later in life is associated with an increased risk of dementia.
A 2023 study reported that adults over the age of 60 who lost just 1% of their slow-wave sleep annually were 27% more likely to develop dementia. The analysis also found an even stronger association with Alzheimer’s disease, the most common form of dementia worldwide.
What Slow-Wave Sleep Actually Does
Slow-wave sleep, commonly referred to as deep sleep, is the third stage of the typical 90-minute sleep cycle and generally lasts between 20 and 40 minutes. During this phase, brain waves slow significantly, heart rate and breathing decrease, and blood pressure falls, allowing the body and brain to recover.
This stage of sleep helps repair muscles and bones, supports immune function, and prepares the brain for learning and memory formation the following day. Previous research has shown that greater amounts of slow-wave sleep are associated with better memory performance, even among individuals who already exhibit brain changes linked to Alzheimer’s disease.
Inside the Long-Term Dementia Study
The findings come from 346 participants enrolled in the long-running Framingham Heart Study in the United States. All participants underwent two overnight polysomnography assessments, the first between 1995 and 1998 and the second between 2001 and 2003, with approximately five years separating the two evaluations.
None of the participants had dementia at the time of the second sleep assessment, and all were over the age of 60 by 2020. Researchers subsequently monitored participants for up to 17 years, through 2018, to determine who developed dementia while comparing changes in each individual’s proportion of slow-wave sleep over time.
Deep Sleep Loss and Dementia Risk
Across the study population, the proportion of time spent in slow-wave sleep steadily declined after age 60. The most rapid decline occurred between the ages of 75 and 80 before eventually stabilizing. During the 17-year follow-up period, 52 participants were diagnosed with dementia, enabling researchers to examine the relationship between sleep changes and later cognitive outcomes.
For every 1-percentage-point annual decrease in slow-wave sleep, the risk of developing any form of dementia increased by 27%. When the analysis focused specifically on Alzheimer’s disease, the associated increase in risk rose to 32%, suggesting that deep sleep may be particularly important in relation to this condition.
Genetics, Heart Health, and the Brain
The Framingham Heart Study also collects information on additional health indicators, including hippocampal volume, a brain region essential for memory that undergoes early shrinkage in Alzheimer’s disease. Participants with lower levels of slow-wave sleep were more likely to have cardiovascular risk factors and to use medications known to interfere with sleep.
Researchers also examined the APOE ε4 allele, a well-established genetic risk factor for Alzheimer’s disease. They found that carrying APOE ε4 was associated with a more rapid decline in slow-wave sleep. In contrast, overall brain volume loss did not appear to account for the reduction in deep sleep observed over time.
Why Deep Sleep May Matter
Lead author Matthew Pase of Monash University in Australia noted that slow-wave sleep supports the aging brain through several mechanisms. Previous research suggests that this sleep stage helps clear metabolic waste products from the brain, including proteins that accumulate in Alzheimer’s disease.
By supporting this nightly clearance process, deep sleep may help reduce the accumulation of toxic proteins and inflammatory processes that can damage brain cells. However, the researchers caution that their findings demonstrate an association and do not establish that sleep loss directly causes dementia.
Cause or Consequence of Dementia?
One important question is whether reduced slow-wave sleep contributes to dementia development or whether early brain changes associated with dementia disrupt deep sleep. It is possible that both mechanisms are involved, with poor sleep accelerating disease progression in susceptible individuals.
The authors emphasize that only randomized clinical trials and experimental studies can determine whether improving slow-wave sleep reduces dementia risk. Future research may investigate whether targeted sleep interventions can delay cognitive decline among people at elevated risk.
Practical Steps for Better Sleep
Despite the remaining uncertainties, experts agree that maintaining healthy sleep habits is a practical strategy for supporting brain health. Common recommendations include maintaining consistent sleep and wake schedules, limiting caffeine and alcohol consumption later in the day, and keeping the sleeping environment dark, quiet, and cool.
Some studies suggest that regular physical activity, weight management, and treatment of sleep disorders such as obstructive sleep apnea can increase the proportion of time spent in deep sleep. Although no intervention is guaranteed to prevent dementia, preserving slow-wave sleep may become an important component of broader risk-reduction strategies.
The study, published in JAMA Neurology, adds to a growing body of evidence linking sleep quality to long-term cognitive health. As populations continue to age and dementia prevalence increases worldwide, understanding and protecting deep sleep may become an increasingly important focus of both clinical practice and public health policy.
