A study published in the Journal of Clinical Periodontology suggests that a fast-mimicking diet (FMD) may help reduce inflammation in people with periodontitis when combined with standard dental treatment. Researchers found that participants following the diet showed lower levels of inflammatory markers in both their blood and gum tissue, although the dietary approach did not lead to greater short-term clinical improvement in gum disease.
Periodontitis is a severe form of gum disease that damages the tissues and bone supporting the teeth. Increasingly recognized as a chronic inflammatory condition with links to overall health, it has prompted researchers to investigate whether anti-inflammatory dietary approaches could complement conventional periodontal care.
Researchers Investigated a Fast-Mimicking Diet
A fast-mimicking diet is a structured eating plan that significantly reduces calorie intake for several consecutive days while still allowing limited food consumption. Unlike complete fasting, the diet is designed to trigger many of the same metabolic and cellular responses associated with fasting. Previous studies have suggested that this approach may reduce inflammation and improve markers of metabolic health.
Because periodontitis is driven by an exaggerated inflammatory response to bacterial plaque, the researchers proposed that lowering inflammation throughout the body might also improve inflammatory processes in the gums. They investigated whether combining a fast-mimicking diet with professional periodontal treatment could influence both local and systemic inflammation.
Small Clinical Trial Measured Inflammatory Changes
The study included 28 adults diagnosed with periodontitis. At the beginning of the trial, all participants underwent intensive deep cleaning, a standard treatment that removes bacterial plaque and tartar from beneath the gums. Participants were then randomly divided into two groups.
One group followed the fast-mimicking diet, while the other continued their usual diet without calorie restriction. During each cycle, participants consumed approximately 1,100 calories per day for the first two days, followed by around 750 calories per day for the next three days. They resumed their normal diet on days six and seven. This schedule was repeated three times over three months, followed by another three months of observation.
After six months, participants following the fast-mimicking diet showed lower concentrations of inflammatory markers in both their blood and gingival crevicular fluid, the fluid found in the space between the teeth and gums. These findings suggest the diet reduced inflammation locally within the gums as well as throughout the body. Participants in the control group did not experience the same reductions.
Gum Disease Outcomes Were Similar in Both Groups
Although inflammation decreased in the fast-mimicking diet group, the researchers did not observe meaningful differences in the clinical improvement of periodontitis over the six-month study period. Measures such as reductions in periodontal pocket depth and other indicators of gum healing were comparable between the two groups.
The authors therefore emphasize that the fast-mimicking diet should not be considered a replacement for established periodontal treatment.
Lower Inflammation Could Still Be Important
Even without greater short-term improvements in gum health, reducing inflammation may still have broader health benefits. Periodontitis has been associated with an increased risk of cardiovascular disease, stroke, and possibly cognitive decline, potentially because of long-term inflammatory and vascular effects throughout the body.
The researchers suggest several biological mechanisms that could explain the observed reductions in inflammation. Fasting and calorie restriction have previously been shown to reduce oxidative stress, a process that contributes to inflammation by damaging cells and DNA. The fast-mimicking diet also reduces consumption of highly processed foods and added sugars, which may promote inflammation and alter the balance of bacteria in the mouth.
According to the researchers, lifestyle changes may become an important complement to brushing, flossing, professional cleanings, and other conventional periodontal treatments. However, they stress that the current findings are preliminary and should be interpreted cautiously.
Larger Studies Are Needed
The researchers acknowledge that the study was relatively small and not designed to provide definitive evidence of clinical effectiveness. Larger studies involving more participants and longer follow-up periods will be needed to determine whether the reduction in inflammatory markers ultimately leads to improved gum attachment, fewer missing teeth, or lower risks of other chronic diseases.
Future research may also identify which patients benefit most from the fast-mimicking diet and explore alternative approaches for people who cannot safely restrict calories, including individuals with certain metabolic disorders. Researchers are also interested in therapies that reproduce the biological effects of fasting without requiring strict dietary changes, as well as more flexible dietary strategies that retain anti-inflammatory benefits while being easier to maintain.
For now, experts recommend that people with periodontitis continue following well-established measures, including good oral hygiene, regular professional dental care, and a balanced diet rich in vegetables, whole grains, and minimally processed foods. Anyone considering a calorie-restricted eating plan should first discuss it with a healthcare professional or registered dietitian.
