Researchers Identify The Amount Of Strength Training Associated With Lower Mortality Risk

Regular strength training is increasingly recognized as a powerful contributor to long-term health, not merely a way to build muscle. A new analysis of long-running U.S. health studies suggests that performing a modest amount of resistance training each week is associated with a lower risk of premature death.

The research pooled data from nearly 150,000 nurses and other healthcare professionals who were followed for up to 30 years. Every two years, participants reported how often they engaged in strength training and aerobic activities such as walking, cycling, or swimming.

Over the three decades of follow-up, nearly 36,000 participants died, allowing researchers to compare different exercise patterns with overall mortality risk. They found that adults performing approximately 90 to 120 minutes of strength training per week had about a 13% lower risk of death from any cause compared with those who performed no strength training.

How Much Strength Work Is Enough?

The analysis identified a clear optimal range rather than a simple “more is better” relationship. Approximately one and a half to two hours of strength training per week was associated with the lowest overall mortality risk, particularly for cardiovascular and neurological causes.

Compared with individuals who performed no resistance exercise, those within this optimal range had a 19% lower risk of death from cardiovascular disease. They also demonstrated a 27% lower risk of death from neurological disorders, including dementia and related conditions.

Beyond approximately two hours per week of strength training, the benefits appeared to plateau. Additional time spent performing resistance exercise did not further reduce mortality risk, suggesting diminishing returns at higher training volumes.

For cancer, the pattern differed slightly. Modest amounts of strength training, less than one hour per week, were associated with lower cancer mortality, but higher volumes did not appear to provide additional benefits. Researchers caution that further investigation is needed to better understand this relationship.

Strength and Aerobic Exercise Together

The study also reinforced the importance of aerobic exercise for longevity. Meeting or exceeding current recommendations of at least 150 minutes of moderate-intensity aerobic activity per week was associated with a 26% to 43% lower risk of death when considered independently.

The greatest benefits were observed among participants who combined regular aerobic exercise with one to two hours of weekly strength training. This combination was associated with approximately a 45% lower risk of death compared with individuals who performed neither type of exercise.

In other words, aerobic activity accounted for much of the reduction in mortality risk, while strength training served as a powerful complementary factor. The findings support current public health guidelines encouraging adults to engage in both aerobic and muscle-strengthening activities.

Why Muscle Matters for Longevity

Researchers suggest that the association between strength training and longevity is rooted in the many physiological functions of skeletal muscle. Muscle is metabolically active tissue and plays a critical role in regulating blood glucose levels, an important factor in the development of type 2 diabetes and cardiovascular disease.

After meals, a substantial proportion of glucose entering the bloodstream is absorbed by skeletal muscle under the influence of insulin. Healthy muscle tissue helps remove glucose efficiently, either by using it for energy or storing it as glycogen, rather than allowing it to contribute to fat accumulation and metabolic dysfunction.

Skeletal muscle also functions as an endocrine organ, releasing hormone-like signaling molecules known as myokines during contraction. These molecules travel throughout the body and influence organs such as the liver, brain, blood vessels, and adipose tissue, often helping to reduce the chronic inflammation associated with cardiovascular disease and cancer.

Resistance training has also been shown to improve blood pressure, maintain arterial flexibility, and support healthier lipid profiles. Together, these effects contribute to a lower risk of cardiovascular disease, which remains one of the leading causes of death worldwide.

Protecting the Brain and Independence

The apparent protective association with neurological mortality, including dementia-related deaths, is a relatively new area of investigation. Improved cerebral blood flow, better glucose regulation, and reduced systemic inflammation are all believed to contribute to long-term brain health.

Emerging evidence also suggests that regular strength training may promote structural and functional adaptations within the brain. These changes may include improved connectivity and resilience in regions involved in memory and executive functioning, although additional research is required to confirm these effects.

Beyond disease prevention, maintaining muscular strength is strongly associated with quality of life in older adulthood. Greater strength reduces the risk of falls and serious fractures and helps delay frailty, allowing individuals to maintain independence for longer.

Simple measures such as grip strength, which reflects overall muscular strength, have repeatedly been shown to predict mortality risk. In several large international studies, grip strength has even outperformed blood pressure as a predictor of premature death.

Limits of the Evidence and Practical Advice

The authors emphasize that this was an observational study and therefore cannot establish a causal relationship between strength training and longer life. Individuals who engage in resistance training may also have healthier diets, better access to healthcare, or other beneficial lifestyle habits, even though the analysis adjusted for many of these factors.

Another limitation is that exercise participation was self-reported, and the study did not assess workout intensity. Different forms of resistance training, ranging from light bodyweight exercises to heavy weightlifting, were grouped together within the same category.

Despite these limitations, the amount of strength training associated with lower mortality risk is achievable for most adults. Short resistance-training sessions performed two or three times per week can be sufficient to engage all major muscle groups when combined with regular walking, cycling, or other forms of aerobic activity.

Experts note that a gym membership or heavy equipment is not required to obtain these benefits. Bodyweight exercises, resistance bands, and light dumbbells can all help build and maintain muscle mass, particularly when used consistently over months and years.

Current public health guidelines in many countries already recommend at least 150 minutes of moderate-intensity aerobic activity per week, along with muscle-strengthening activities on two or more days weekly. This new long-term analysis strengthens those recommendations by linking them to substantial reductions in the risk of premature death.
