Flavoured e-cigarettes have long been marketed as a smoother and more enjoyable alternative to traditional smoking, with cooling “ice” varieties becoming especially popular. While regulators have largely focused on nicotine and addiction, researchers are increasingly investigating whether flavouring compounds themselves may pose risks to cardiovascular health.

Many cooling vape products rely on menthol or synthetic cooling agents such as WS-3 and WS-23. These compounds do not actually reduce the temperature of the aerosol. Instead, they activate TRPM8 receptors, the body’s cold sensors, creating the sensation of coolness. By masking throat irritation, they may encourage users to inhale more deeply and more frequently.

How Cooling Agents Affect The Body

TRPM8 receptors are found not only in the mouth and airways but throughout the body, including in blood vessels and nerve tissue. Their widespread distribution has led scientists to question whether menthol and similar compounds might trigger broader physiological effects. What was once considered a simple flavouring ingredient is now being studied as a potentially active contributor to cardiovascular changes.

Animal studies have begun to shed light on these concerns. In a recent experiment involving mice, exposure to menthol-flavoured e-cigarette aerosols resulted in higher heart rates and elevated blood pressure compared with exposure to non-menthol aerosols. Researchers also observed an increase in ventricular premature beats, a type of irregular heartbeat that may signal added stress on the heart’s electrical system.

The study further revealed elevated levels of epinephrine, the hormone associated with the body’s fight-or-flight response. Some disturbances in heart rhythm regulation persisted for weeks after exposure ended, raising concerns that the effects may extend beyond short-term stimulation. Researchers are also examining how flavouring compounds affect blood vessel health. Regulators in the United States and Europe have introduced restrictions on flavoured e-cigarettes in an effort to reduce youth vaping. However, most regulations focus on flavour categories rather than individual chemical compounds. While menthol has attracted considerable attention because of its long history in conventional cigarettes, synthetic cooling agents often fall outside specific regulatory frameworks.

Health authorities, including the U.S. Food and Drug Administration, continue to highlight significant uncertainties surrounding the long-term effects of vaping on both the heart and lungs. Human studies have already shown that vaping can temporarily increase blood pressure and reduce arterial flexibility. However, separating the effects of nicotine from those of flavouring compounds remains challenging.

A major concern is that many flavouring substances were originally approved as food additives and are therefore often assumed to be safe. Experts emphasize that safety when eaten does not automatically mean safety when inhaled. The lungs provide a rapid pathway into the bloodstream and expose delicate tissues directly to inhaled chemicals. New flavour formulations also enter the market faster than researchers can fully evaluate their potential health effects.

What Consumers And Policymakers Should Know

Current evidence does not prove that cooling vape flavours directly cause heart disease in humans. However, it increasingly challenges the assumption that these additives are biologically inactive. Experimental studies have identified measurable effects on heart rhythm, blood pressure, and vascular cells, prompting calls for closer scrutiny of menthol and synthetic cooling agents.

Public health experts warn that adolescents and young adults may underestimate the risks associated with flavoured vaping products. The cooling sensation can make aerosols feel smoother and less irritating, potentially disguising the presence of nicotine and other harmful substances.
